Your views and feedback are of great importance to the practice and help us to ensure that we are providing the best quality service and information to our patients.

To obtain the necessary data and opinions, we are planning to carry out a patient satisfaction survey each year, usually in February but sometimes at other times, to see how we are doing. To achieve this, we would like to create patient participation group the members of which have given their consent to be contacted periodically by letter, or preferably, by email. It is likely that the information we require will take only a few minutes to provide.

You can agree to participate in this process by completing a form in the surgery and handing it in at the main reception desk. This form is also available online below. Your views are always greatly appreciated and do help us to make improvements and tailor our services to your needs.

From time to time, we intend to publish some results on this website and we may refer to views expressed by patients. However, we will not publish the names of any patients or staff.

If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.
